Benefits Form Please follow the following procedures
to obtain a Plus+1 Gull Card. This card will give you access to the
following benefits.

- The employee must bring their ID ( to verify that they are
employed at SU) when they accompany the Plus+1 friend/family member
to the Gull Card Office located in the Commons Bldg Rm 036.
- The employee will be asked to complete a very short
Benefits Form which will indicate their friend or family member
is their designated
Plus+1 member.
- The Plus+1 member will simply need to provide their Social
Security Number. (Currently this is the only way we are able to
track non-employee or non-student individuals in the system but this
procedure is expected to change within the next year.)
- To add dependent children, the current process will remain
unchanged. The employee will affirm that the child(ren) is their
legal dependent(s) and a Gull Card is issued.
